Major depression questionnaire

Instructions: Below is a list of questions that relate to life experiences common among people who have been diagnosed with depression. Please read each question carefully, and indicate how often you have experienced the same or similar challenges in the past few months. This scale is not designed to make a diagnosis of depression or take the place of a professional diagnosis. Answer each of the items below about how you have felt and behaved during the past week.

Patients may complete questionnaires at baseline and at regular intervals (eg, every weeks) at home and bring them in at their next appointment for scoring or they may complete the questionnaire during each scheduled appointment.
